What are the most common causes of back pain?

Muscle and ligament strain — from poor mechanics, sudden movement, or prolonged posture — accounts for the majority of acute back pain. Disc herniation, degenerative disc disease, facet joint arthritis, and spinal stenosis are important structural causes. Serious causes (fracture, infection, cancer) are uncommon but require consideration.

When does back pain require imaging?

Most acute back pain does not require immediate imaging — it rarely changes management for non-specific low back pain. Imaging is indicated for red flags: trauma, significant osteoporosis history, fever with back pain, neurological deficits (weakness, numbness, bowel or bladder changes), or pain not improving after 4–6 weeks of appropriate management.

Is bed rest recommended for back pain?

No — evidence consistently shows that staying active produces better outcomes than bed rest. Gentle movement, walking, and continuing normal activities as tolerated accelerate recovery. Prolonged rest leads to muscle deconditioning and often worsens and prolongs symptoms.

When should I see a physical therapist for back pain?

Early PT referral — within the first 2–4 weeks for persistent symptoms — produces better outcomes than delayed referral and significantly reduces progression to chronic pain. PT addresses the underlying mechanical contributors and provides an active recovery program rather than passive symptom management alone.

What side effects are associated with back pain treatments?

NSAIDs carry GI and renal side effect risks with regular use. Muscle relaxants (cyclobenzaprine) cause significant drowsiness. Opioids are not recommended for acute non-specific back pain due to poor evidence and addiction risk. Epidural steroid injections may cause temporary pain flare and blood sugar elevation.
